When you do a progress note, you must also attach a verification of the session.
As a condition of Medicaid and insurance reimbursement, every billable service must be supported by both accurate documentation and a valid verification of the service provided. Session verifications help demonstrate that the service occurred as documented, at the date, time, and duration billed. These requirements are established by Medicaid, managed care organizations, and regulatory agencies, and are routinely reviewed during audits and investigations.
Please understand that the purpose of obtaining verifications is not because the agency distrusts its clinicians. Rather, verifications serve as an important compliance safeguard that protects our clients, our staff, and the organization by providing objective evidence that services were delivered appropriately. Proper verifications help prevent billing disputes, support the integrity of the clinical record, and ensure continued compliance with state and federal regulations.
Every progress note must be accompanied by a legitimate verification obtained in accordance with agency policy. Verifications must accurately reflect the actual service provided and should never be completed for services that did not occur or for a duration that does not match the actual time spent with the client.
Inna will not be able to process your note without this.
What kind of verifications are acceptable?
- any paper-verification form as long as it follows the example (for in-person) (see A1 below)
- the client can sign the progress note itself (see A2 below)
- a screenshot of the Doxy.me date/time log (see A3 below)
- a text message showing the client's phone number (or partial phone number), showing that the client verified. Examples: Lisa was here on 5/10 for 1 hour, or Lisa was here on 5/10 from 2:10 to 3:10. (see A4 below)
- a screenshot of the client during the session, showing the computer's date/time (the time on the screenshot must fall between time spent in the session - e.g. if your session was from 2:10 to 3:10, then the time shown must fall in between those times. (see A5 below)
- our online verification form (the verification must show the client's email, and a copy of the signed form will be automatically emailed to the client after signing - only the client's email is acceptable to put in this form) (see A6 below)
